Infarction - tissue death caused by loss of blood supply

My uncle Steve lives in Lubbock. We found out yesterday morning that he was in the hospital. He suffered a major heart attack and was unconscious.

Immediately, to me, this meant clot and not a fatty build-up in a coronary artery. Today, this was confirmed. He is suffering from infarction of the heart, lung and brain due to a clot that broke free from somewhere -- my guess would be the legs from a general likelihood type thing.

I'm guessing he's on coumadin or heparin as a general practice, but if he were to suffer another heart attack right now, he would not be likely to survive resuscitation due to the damage to his heart already.

Yesterday, his EEG gave a brain activity rating of 0-1 on a 9 point scale. Today, it gave a 3-4. 3-4 is still considered a vegetative state. He breathes on his own, but is unconscious. Anything under an 8 is considered not able to operate autonomously.

Because of the initial improvement, a DNR has not been issued. Aunt Pam would like to see what an EEG shows tomorrow before making the decision.

I'm hoping for the best, but I don't know if I could be prepared for the worst. Best case is a slow recovery, cognitive and occupational therapy. The liklihood of that is very low to the point that the doctors don't really consider it a possibility.

The worst would be a persistent vegetative state, no closure. That would be very very difficult for my aunt Pam and my uncle Jon. My grandmother doesn't have enough persistence of cognition for it to be a big effect on her, but my one uncle already has the weight of that on him. He's not married, one brother dead, father dead, mother senile - just very difficult for him. I honestly don't know Pam and her family very well. We see each other an average of one holiday per year, but regardless of her available family support, this has to be very very hard on her.

My sister is the relay point for us, so my uncle Jon doesn't have as many places to keep updated, and because she generally just happens to be a more reliable information relay than me (less easily distracted).

I'm going to be in Amarillo for work next week, which is only 2-3 hours from Lubbock. I honestly can't remember if I've ever been there. Definitely not during my sentient life.

That's scary. I will hope for the best, too.

Sorry to hear about this. The circumstances are very unfortunate. I can't imagine how difficult it must be for the family to make these choices. My thoughts are definitely with you and your family.
